Please refer to the local campaign timeline page for deadline dates pertaining to toy applications before completing this form.

This form only accepts Toy requests from Non-Profit Agencies and Organizations. Unless otherwise noted, the campaign accepts toy requests for children in the age groups provided.

This request form is for 501(C)3 Charitable Organizations to request toys. Please put your EIN number as it appers on your 501(C)3 form, its should look like this (XX-XXXXXXX or XXXXXXXXX).

Please ensure all blocks are completely filled out.

Upon completion of your online application, please send a PDF copy of the 501(c)(3) IRS verification document and "Consumer's Certificate of Exemption" from the Florida Department of Revenue as well as an Excel Spreadsheet with the children's gender, names, ages and address to west.palm.beach.fl@toysfortots.org

Failure to do the so could result in disapproval of the application.

All requests must be submitted NO LATER THAN November 13th, 2026. Requests after this date will be disapproved.

Orders are filled with the toys we have on board when the order is requested.
